What does this form do?
Filling out this form provides your contact information, appointment preferences, and consent for a Skylark Team Member to contact you to schedule an appointment.
How long does it take to get a call back?
We promise to reach out as soon as we can – usually within a few hours of receiving the request, depending on if our team is busy serving other clients. Keep in mind if a request is submitted outside of business hours, we will call you during our next normal business day.
Do I have to make an appointment?
No. Appointments are not required but are recommended. Having an appointment guarantees you will be seen at the scheduled time, while appearing as a walk-in does not offer that guarantee.
What do I need to bring?
Please bring a photo ID — either a driver’s license or a school ID. If you do not provide a photo ID, there are certain medical services we are unable to provide as they require identification.
I am under 18. Do I need a parent’s permission?
You do not have to have a parent’s permission, but we do encourage you to involve and seek advice from your parents or another trusted adult.
Will you tell anyone?
Our services are entirely confidential. We will not tell anyone of your visit unless required to do so by law (i.e. discovery of abuse or neglect of a minor).
